Alvin High School has an insurance policy on all athletes, and it serves as secondary insurance coverage.

When the Athletic Trainer has evaluated an injury and determined that it needs to be seen by a physician, they may recommend a Sports Medicine specialist.  Additionally, they must complete the appropriate school insurance form. 

To ensure that you stay in our network or for after-hours insurance information, you may contact Amber Fragoso at 815-592-5599.  She can direct you to both physicians and emergency services.  (Please limit calls between 9AM and 9PM)
